I have a markdown document open but can't figure out how to use the Speed Read function. I had to look in the help to find it in the gear menu (why not in a main menu?) but it's greyed out and not selectable. Is there something I have to do to enable it? The help didn't suggest anything to me.

This is resolved in 3.0.17, which will be out today for Paddle and Setapp customers.
That shouldn't be happening, I'll look into it right away. Which platform are you on (MAS, Paddle, Setapp)?
I'm on Paddle with a Lifetime license.
Once I enabled "Sync Speed Reading with Scroll Position" in Settings->Preview it worked. It wasn't clear to me that this was required to use the feature; if that's not the case then maybe this is a bug?
Thanks for this report, I have it fixed for the 3.0.17 update (3.0.16 is out now). The menu item was incorrectly bound to "Enable mini map" for some stupid reason, and the preferences around it were jumbled.
That's a good lead. No, it shouldn't be required at all, I'll trace that path.
