Brett, following is a URL to an image I have in a document:
http://f.cl.ly/items/1v1F413J3s0O0H3N2b2M/Untitled.png

When I have the Custom Markdown Processor enabled, the image renders in preview. When I do not have the Custom Markdown Processor enabled, the image does not render.

Sorry for missing something obvious, I am sure.

Your help is always appreciated.

Sorry, here is the URL without the !, that will hopefully get the URL through to you. I do have the ! preceding the following in my document, however.

That syntax is not recognized by MultiMarkdown. (url "optional title" width=350 height=250) is mmd syntax.
