What is this red dash that is always shown on the top left corner of my Markdown file and how do I remove it?

It's the point of the last edit. It doesn't go away unless there are no
changes to the document on a refresh.

Curious: Even when I do a refresh, the mark doesn't go away. Should it?

The mark will always show the last place that was edited, even after a refresh. If you don't want the mark at all, it can be disabled by checking Scroll to Edit and unchecking Show Edit Marker in Preferences->Preview. (You can then uncheck Scroll to Edit if you don't want that feature).
