Default Fixed TOC
Is there a way to either:
- Display fixed table of contents as default when opening files.
- Default to fixed table to contents instead of popup (to avoid needing to click the little button in popup each time)

It's not currently available a persistent option, but I would
considering adding it. It was originally designed for full-screen mode
(and is the default there), but when I added the option to switch out of
it, windowed mode also got the option to switch into it. I'd probably
just make that option sticky instead of adding a preference.
