How do I export the TOC?
When viewing the file in Markd there is a TOC button (bottom right - 4th from the outside) and it keys off headers (# ## ###, etc..)
In preferences I have enabled Print table of contents, but it never generates in the export

You can use Command ⌘ instead of Control ^ on Mac

Looks like I need to update the documentation, you now need to include
the Table of Contents in the document for it to be exported, using
either Marked's syntax, or MultiMarkdown syntax:

This will export in both print and HTML, but be aware that due to
limitations in the current version of WebKit, exported TOC in PDFs will
not be clickable for navigation.
