<p>I've been doing some more R&D! The "double file opening problem" is ONLY happening with files opened from the "Downloads" directory.</p>
<p>If I open a file from my "Documents" or "iCloud Drive" directories then only ONE instance of the file is opened.</p>
<p>This probably makes the problem even more difficult.</p>
<p>I do most of my initial writing from the "Downloads" directory so that's why I noticed it and reported it.</p>
